A blood test can now spot Alzheimer's disease years before your memory fails. In July 2026, researchers at Washington University School of Medicine demonstrated that circular RNA biomarkers predict who will develop symptoms with unprecedented accuracy—outperforming every existing test. Dr. Carlos Cruchaga, who led the study, stated: "This study represents the first time we've demonstrated that blood-based circular RNAs can predict progression to symptomatic Alzheimer's disease, and they do so with performance that exceeds our current gold-standard biomarkers." The circular RNA signal increases steadily from the presymptomatic stage—two to four years before symptom onset—until symptomatic Alzheimer's disease.

But the breakthrough is landing at the exact moment when new Alzheimer's drugs cost $26,500 to $32,000 annually, Medicare budgets are strained, and clinical guidelines warn against testing people without symptoms because most can't access the treatments that would make early detection matter. The question isn't whether we can see Alzheimer's coming. It's whether anyone but the wealthy will be able to do anything about it.

What the Science Shows

The research team identified 34 circular RNAs linked to Alzheimer's by sequencing blood samples from 1,221 individuals. Unlike biomarkers that track only amyloid or tau, circular RNAs capture the disease's full biology—neuroinflammation, synaptic breakdown, protein clumping—in a single integrated readout. Eighty-seven percent of the circular RNA signals operated independently of their parent linear mRNA, revealing a distinct regulatory layer.

The circular RNA signature hit an Area Under the Curve of 0.945 for detecting biomarker-confirmed Alzheimer's, compared to 0.877 for plasma pTau217. For identifying who would develop symptoms within five years, the circular RNA model reached 0.870—a 29% improvement over pTau217's 0.676. Combine circular RNAs with pTau217 and you get the highest predictive ability, reaching an AUC of 0.977. The circular RNA levels start diverging two to four years before symptoms, catching presymptomatic changes that amyloid-focused tests may miss.

The team validated their findings across three independent cohorts—two from the Knight Alzheimer's Disease Research Center and one from the A4 study—analyzing more than 3,000 blood RNA samples from Knight-ADRC and 1,700 from A4. The panel showed low predictive power for Parkinson's disease, frontotemporal dementia, and Lewy body dementia, confirming high specificity for Alzheimer's. And circular RNAs can be detected in plasma using standard, inexpensive PCR-based assays, enabling potential population-based screening. Circular Genomics launched its CircPATH platform right after publication, partnering with Washington University.

The scientific achievement is real. What happens next depends entirely on who controls access to what comes after the test.

The Treatment Gap

Only people in early-stage Alzheimer's qualify for current disease-modifying drugs, and faster diagnosis would help patients access them during the time frame when they are most helpful. Dr. Elizabeth Edgerly of the Alzheimer's Association put it plainly: "If you get a diagnosis early, you're actually able to access treatments that you cannot take later on in the disease process."

That argument assumes treatment is within reach. For many Americans, it isn't.

Leqembi costs $26,500 a year. Kisunla runs $32,000. Medicare covers both under Part B, but only if you have a diagnosis of mild Alzheimer's or mild cognitive impairment, you're enrolled in Original Medicare or Medicare Advantage, and your doctor registers your patient data in a CMS-facilitated federal registry. Even then, Medicare typically handles 80% of the cost, leaving patients—or their supplemental insurance—on the hook for the remaining 20%. Without supplemental coverage, that amounts to about $5,300 annually for Leqembi and $6,400 for Kisunla.

And under existing criteria, asymptomatic people don't qualify for treatment at all, meaning testing offers no meaningful benefit and may constitute medical futility.

The Access Problem

Current clinical guidelines recommend against biomarker testing for cognitively unimpaired adults, as the presence of biomarkers does not guarantee the development of clinical Alzheimer's disease, and testing is not ethically justified unless there is a clear benefit to the patient.

Yet Alzheimer's blood tests are already hitting the market. The FDA approved the first one—Lumipulse—in May 2025 for people ages 55 and older with cognitive decline. Insurance doesn't consistently cover these tests, with out-of-pocket costs ranging from $300 to $2,000.

Tests costing hundreds to thousands of dollars followed by drug costs in the tens of thousands annually create a system where early detection becomes a privilege for those who can afford what comes next. Everyone else gets a diagnosis and a wait.

The Downstream Harms

The risks of premature testing go beyond financial strain. Disclosure of biomarker status to asymptomatic individuals can cause psychological distress, anxiety, and premature labeling of Alzheimer's pathology in people who are currently healthy. Alzheimer's stigma can lead to social exclusion, employment discrimination, housing denial, and insurance bias, with current laws like GINA not protecting individuals discriminated against based on biomarker status.

Even for those who can afford testing, accuracy isn't equally distributed. Three of four experimental Alzheimer's blood tests perform differently in Black individuals compared to white individuals. One blood test optimized for Alzheimer's detection showed 90.3% sensitivity in white participants but only 73.7% in Black participants, with a positive predictive value of 87% for whites versus 58% for Blacks. Most biomarker research relies on data from predominantly white volunteers, raising concerns about test validity in diverse populations.

There is no current evidence that the circular RNA-based test has been evaluated for racial disparities. Black patients are 35% less likely to be diagnosed with Alzheimer's or dementia despite having approximately twice the risk of developing dementia compared to white patients. A breakthrough test that replicates existing inequities doesn't serve public health. It extends them into new territory.

The Budget Reality

Approximately 2 million Medicare beneficiaries with early-stage Alzheimer's disease are currently eligible for anti-amyloid drugs but lack routine Medicare coverage due to the Coverage with Evidence Development requirement. CMS recently enabled coverage of FDA-approved Alzheimer's treatments through a low-touch registry, improving access for eligible patients. The Alzheimer's Association advocates for laws requiring insurance coverage of blood-based dementia screening tests through the proposed Alzheimer's Screening and Prevention (ASAP) Act. Ninety-one percent of Americans say they'd want the test if available, primarily to access early treatment.

But expanding screening to millions while covering five-figure annual drug costs would strain Medicare budgets, potentially forcing choices about which patients receive treatment even when early detection identifies them as candidates. The circular RNA test's very accuracy—its ability to spot Alzheimer's years before symptoms—multiplies the number of people who'd qualify for expensive drugs, increasing pressure on a system already struggling to deliver equitable care.

What Fair Implementation Requires

The circular RNA breakthrough offers a window into Alzheimer's disease years before symptoms appear with unprecedented accuracy. Yet it arrives in a moment when treatment remains limited to early-stage patients, costs reach tens of thousands annually, and clinical guidelines question the ethics of testing people who cannot access intervention.

Fair implementation would require not just scientific validation and FDA approval, but deliberate policy choices: insurance coverage for both testing and treatment, price negotiations to make drugs affordable, validation across diverse populations to prevent diagnostic inequity, and honest conversations about what early detection means when intervention remains out of reach.

The question facing medicine and society is whether we can build a system where breakthrough science serves everyone—or whether early Alzheimer's detection will join the growing list of medical advances that deepen rather than close the gap between those who can afford health and those who cannot.
